5 easy Steps.

1.   CLICK THIS LINK to download our supporting information pack (A new page will open).

2.  Carefully read through this pack and confirm that you are eligible to work under PGDs.

3.  Speak with your Principle Dentist/Manager and confirm they are happy for you to work under PGDs.

4.  Complete the online order form and enter all information requested. Your PGDs will be processed within 72 hours.

5.  Once you have received your PGDs, you and your Principle/Manager sign pages 2 and 5 and return back to us.
